“Why Do I Get Cavities Even Though I Have Great Dental Hygiene?”

A frustrated patient recently posted this question to Reddit:

This scenario is a common one: A person follows all the “rules” yet still struggles with decay, while others seem able to stay cavity-free despite a junky diet or less-than-great hygiene. That’s because while diet and hygiene play an outsized role in your oral health, they’re not the only factors in play.

Beyond the Toothbrush: Hidden Factors in Cavity Formation

Here are some other factors that can significantly affect your decay risk, often without you even realizing it:

Saliva quality & quantity. Saliva is your mouth’s natural defense system, neutralizing acids and delivering needed minerals to your teeth. Some people naturally produce less, creating a drier environment where harmful bacteria can thrive. Others’ saliva may not have the right mineral content for remineralizing teeth. Diet, stress, medications, hormonal changes, and certain medical conditions can all affect both the quality and quantity of your saliva.

Oral pH. Your mouth’s acidity level changes through the day, but it should mostly be neutral or slightly alkaline. A more acidic environment favors decay-causing bacteria. This can be affected by factors other than diet – breathing patterns, for instance, or genetic variations.

Oral dysbiosis. Your oral microbiome contains hundreds of bacterial species, and the balance between helpful and harmful microbes is crucial. Some people harbor more aggressive strains of cavity-causing bacteria, while others may have microbiomes that have been disrupted by antibiotics, illness, diet, or other factors. (An ultra-processed diet is especially apt to throw the microbiome out of whack.) Simply removing plaque through brushing and flossing doesn’t address the underlying imbalance.

Differences in tooth structure. Some people’s teeth have naturally deeper grooves and fissures in which harmful bacteria can thrive, enamel that’s less mineralized, or genetic variations that affect how their teeth respond to acids. These structural differences can make some people more susceptible to decay.

Stress & sleep. Chronic stress raises cortisol levels, and this can suppress immune function and change the oral environment in ways that promote bacterial overgrowth. Poor sleep affects the immune response, too, as well as saliva production. Clenching and grinding during stress or sleep can create micro-cracks that provide entry points for harmful bacteria.

Medications & health conditions. Many common medications – from antihistamines to antidepressants – can reduce saliva flow or change its composition. Conditions like diabetes, autoimmune disorders, or hormonal imbalances can also affect your mouth’s natural defenses in ways that promote decay.

An Anti-Decay Diet Is More Than Just “Limit Sugar”

We all grow up hearing variations of “sugar causes cavities,” which is true but also an oversimplification. The relationship between diet and oral health goes much deeper. What matters is how your eating patterns affect the oral environment.

Mineral density & bioavailability. Your teeth are living tissues that are in a constant cycle of demineralization and remineralization. The process requires a steady supply of calcium, phosphorus, magnesium, and trace minerals, as well as the ability to absorb and utilize them effectively. Factors such as vitamin D status and gut health affect this, as can anti-nutrients and mineral blockers. Phytic acid in grains and legumes, for instance, or oxalates in certain vegetables can affect mineral bioavailability.

Hidden sugars & refined carbohydrates. Although you may avoid obvious sugars, many processed foods contain hidden sugars or other refined carbs that bacteria can easily convert to acids. Even “healthy” foods such as granola bars or whole grain crackers can contribute to an acidic environment if eaten frequently.

Eating frequency. It’s not just what you eat but when and how often. Two brothers can both be given the same type and size of muffin for breakfast, but the one who saves his to graze on throughout the day is more apt to get cavities. Grazing throughout the day – or sipping on drinks other than water – keeps conditions in the mouth acidic for extended periods. Saliva needs time between eating and drinking episodes to neutralize acids and start remineralizing your teeth.

Inflammatory foods. Foods that promote systemic inflammation – ultra-processed products, for instance, or industrial seed oils – can compromise your immune system’s ability to manage oral bacteria and may affect saliva quality.

Gut health. Your digestive health directly impacts your oral health. Gut dysbiosis can affect nutrient absorption, immune function, and the composition of your oral microbiome. Poor gut health may leave you deficient in key nutrients for remineralization, regardless of what you’re eating.

The Biological Approach: Addressing Root Causes

As a biological dental office, we don’t just treat decay by just drilling and filling while urging more aggressive hygiene to prevent future caries. (“Caries” is the clinical term for decay.) We aim to understand and address the underlying factors that create an environment in which cavities can easily develop. This might include a comprehensive health assessment and nutritional evaluation; saliva analysis and microbiome support; and stress and sleep optimization.

The goal is real prevention. That requires addressing the whole person, not just their mouth and teeth.

Understanding that cavity prevention isn’t just about what you do to your teeth but creating optimal conditions for oral health throughout your entire body, can be the key to finally breaking the frustrating cycle of recurring decay.

Sometimes the answer isn’t doing more of the same. It’s doing something completely different.
